Name on Back (Optional)
If you do not add a custom name, the athlete’s name will be left as pictured unless indicated otherwise in the Staffer’s Name section down below.
If you would like no name on the back and for it to be left blank please type in the notes at the bottom.
If adding name on the back, select font style you would like. Brush Script will be used by default if no option is chosen. You may use the font that is on the replica unless it is the person’s signature.
Select the location you would like our #BeCoolCrew Staff Member’s name to be located on the jersey. If you have already added your name on back of the jersey, please select Name on Sleeve or No Name. If there is no name on back pictured for this jersey, please choose “No Name on Jersey”.

Jerseys will come will short sleeve as default, unless one of the options below is chosen.
Add or remove inches of fabric from the vertical length of the jersey.
If no option is chosen, diagonal font orientation will be used.
If adding name on the back, select font style you would like. If no option is chosen, Brush Script will be used by default.
Optional Logo Placements
Please note that only 1 bowling ball company brand is available per jersey. Conflicting logos of the same type of brand are not allowed (Turbo / Vise, Dexter / 3G). All logos added to jersey must be accounted for and added to the preferred placement down below. If we do not have the logo available in the drop-down list, please select “Custom Logo”. If you wish to use a Custom Logo, please send the attachment to orders@coolwick.com with your order number or add the name of the Custom Logo in the Additional Notes section.
If adding PBA, PBA50, or PWBA, please be aware of PBA Jersey Specifications. Please add PBA number in the Additional Notes down below. If you are not a member, please put “Amateur” in the notes.
